INSTRUCTIONS TO USE ONLINE
REGISTRATIONS
FOR VOLUNTEERS / PLAYERS
- From your browser page type
www.eayso.com this takes you to a welcome screen. If this is your first
time click the “First Time” icon. If not simply log in as before.
- The next screen is just an information screen that
lets you know what you can do. Click Next
- The next screen shows you what types of buttons and
icons you will have throughout the registration process. Click Next.
- This screen gives you three selections (Parents)
(Volunteers) (Youth Volunteers) select one and click next.
- On this screen fill in the appropriate information
then click next.
- This screen asks you for an email address, password,
secret question and answer. If you do not have an email address then you can
simulate on by using any combination of letters or number. It should at
least have the @ sign somewhere in the middle of the word/words and be
followed by a .com, or .net. (Example: john@doe2.com.You can also select the
“do this later” icon and move on. Select next when completed.
- Find a Region screen: select the zip code block
(89406) and select search. Fallon only has one region (360), it is already
selected press next the start your application.
- Volunteer Application page, be sure all blocks with a
red * are filled out. These are mandatory
blocks. Look at the upper left of the application form it will have page 2
of 7.
- On page 3 of 7. This page asks you what you want to
volunteer for. Fill all required information and press next
- Page 4 of 7 is the personal and professional reference
page. These references must be different. Complete and press next.
- Page 5 of 7 is the disclosure page. Any “yes” answers
must be explained. Complete and press next.
- Page 6 of 7 is the waiver / consent page. Please read
carefully and check box, type in name and date and press next.
- Your application is ready to be printed, click on
printer icon and following printing instructions. Once you have printed the
form either print it two more times or make two extra copies. One will be
for your records, and two copies go to the CVPA (Pete Gagliardo) Phone #
